Please feel free to set up an appointment to visit our facility if you would like and to have any of your questions answered.

At the initial appointment, please have the following prepared:

1. Completed Child History Questionnaire.
2. If child is mandated services through CPSE or CSE,

   please bring in the most current IEP and evaluations.
3. If you will be receiving reimbursement through your insurance, please bring in a doctor's prescription for occupational therapy services, including child's diagnosis and frequency of services noted. In addition, please remember to bring in the insurance card.
